NOMINATION FORM

The School Age NOTES Foundation is proud to announce the annual national Quest for Excellence award to recognize outstanding contribution of school-age care providers.

One award will be made each year. The recipient will receive one year’s membership to the National AfterSchool Association (NAA), a one-year subscription to School-Age NOTES newsletter, paid registration to the annual NAA conference, as well as airfare to the conference and hotel accommodations for three nights. The recipient will also be recognized at the conference, and must be able to attend.

The selection committee will review each nomination for specific examples of exemplary practice, commitment to continuous improvement, and a professional approach to his/her work with children. The committee is a diverse group of school-age care professionals from across the country.

Eligibility Requirements: Staff who have worked directly with children in an after school program for at least two years are eligible for the award. Candidates must work a minimum of ten hours a week as an employee of a school-age care program that meets five days a week. The program can be based in a school, neighborhood location, military base or private program. A supervisor, parent, board member or teacher, etc., must submit the application on behalf of the nominee.

Please answer the following questions:

Describe the nominee’s accomplishments in the after school/school-age care field (i.e. how this person contributed to success of programs, projects, activities; what impressed you, etc.

Describe how the nominee’s achievements or leadership have helped to benefit children and/or other after school/school-age care professionals. Include specific examples.

How has this individual demonstrated a personal commitment to the field? Discuss ways in which this person has worked to elevate professionalism in after school/school-age care.
